A case manager works alongside you, your family, legal team and treating professionals to assess your needs, create a tailored rehabilitation plan, and make sure therapies, equipment, care and community services are in place and working well together. The aim is to reduce stress, remove barriers and maximise your recovery, independence and quality of life throughout the rehabilitation and litigation journey.
Our case management service is suitable for adults and children following serious or catastrophic injury, including brain injury and complex orthopaedic trauma, whether you are at the very start of a claim or your case has already settled. We offer clear communication, regular reviews and flexible support, so you always know what is happening and who is involved in your rehabilitation.

Immediate Needs / INA reporting to inform early rehabilitation, and application of the Rehabilitation Code
Ongoing rehabilitation case management to support quantum, liability and condition and prognosis evidence through clear, longitudinal reporting
Coordinating multidisciplinary rehabilitation (therapy, neuropsychology, vocational rehab, pain and fatigue services) with defined goals and outcomes
Sourcing, commissioning and monitoring care, support workers and specialist residential placements, with costed recommendations and review
Identifying and evidencing needs for accommodation, housing adaptations, equipment and assistive technology
Liaison with statutory services, Occupational therapy, education and employers to support return to education/work and community integration, reducing the risk of placement or package breakdown
